Acknowledgments (initial each)

I understand that I am responsible for backing up my data before service.

I understand that data loss/corruption may occur and data recovery is not guaranteed.

I authorize NVSP IT to access, test, diagnose, and operate my device as reasonably necessary.

I understand services are provided as-is, with no guaranteed outcome.

I understand NVSP IT is not responsible for pre-existing device conditions.

I understand third-party vendors or subcontractors may be used if necessary.

I understand payment is due upon completion unless otherwise agreed in writing.

Privacy & data handling

NVSP IT takes reasonable measures to protect customer privacy. Access to data is limited to what is necessary to perform services. Data may be temporarily retained for service, billing, warranty, or legal purposes and may be securely deleted after service completion. Absolute security cannot be guaranteed.

I acknowledge and accept NVSP IT’s privacy, data retention, and breach notification practices in accordance with Minnesota law.

By signing below, I acknowledge that I have read, understand, and agree to the NVSP IT Device & Data Service Agreement (Minnesota), which is incorporated by reference into this intake form.
