  • Your device may change condition during service, including cosmetic damage, reduced functionality, or complete failure due to repair risks or pre-existing issues.
  • Diagnostic fees may apply even if you decline repairs.
  • Please remove personal items and accessories unless they are needed for service or listed below.
  • Devices not claimed within 30 days after completion notice may be considered abandoned.

Submitting this intake constitutes electronic acceptance of the NVSP IT Service Agreement.

By submitting this intake you authorize NVSP IT to diagnose and service the device described above as needed for the requested work.

Pop-up service spots: if you choose a pop-up drop-off/pick-up option, locations and times vary and will be confirmed by NVSP IT.

Assessment, diagnostic, and labor charges are non-refundable once work has begun. Special-order parts, software, licenses, and third-party services are non-refundable once purchased or provisioned.
